How can I download and save the pdf documents?

 

Make sure you have Acrobat Reader software installed on your computer. When you click on the link for a pdf document, in the new window there should be a diskette icon below your browser menu and icons. Click on this button and save the document to the selected folder on your computer.

 
My program provides emergency shelter and housing counseling. Under OCAP guidelines, are these considered Housing or Supportive Services?
 
These are considered Housing.
 
My program has an internal evaluation instrument we print out annually to monitor our program effectiveness, as well as a booklet prepared by a consultant who evaluated our program. Should we attach these to our proposal?
 
Yes please.
 
For the goals section of my narrative, should I include program goals for the entire program, or only those for the OCAP funded portion of the program?
 
Please include goals for the entire program. Also include outcomes for the entire program.
 
On my program budget, is it acceptable to use positions as personnel line items, or do I need to list staff names who will fill those positions?
 
Positions are fine.
 
Can I add a line item to our grant budget for Administrative costs?
 
Our grant does not allow administrative costs as a rate. Only direct costs are allowed, under the line items in the budget template.
 
My organization receives a lot of monetary support from individuals. How can I include these in my budget?
 
These would be considered individual donations. You can include this type of income on the Prior Year Secured Funding form for 2006 donations. If these funds will be used for your program costs, you can include them as part of your match on the program budget.
 
Can OCAP grant funds be used to pay for a public service announcement or advertising?
 
Yes, as long as the purpose of the advertising is for outreach and not for fundraising.
 
Can an agency apply alone and as part of a collaborative?
 
Yes, as long as two separate proposals are submitted.
 
How do I complete the Schedule D Ownership Interests section if my organization is a non-profit or public agency?
 
Enter N/A for Ownership Interests, but complete the other sections of the form.
 
Schedule D Section III says "Prime Only." What does this mean?
 
This section is intended only for prime contractors (grantee), as opposed to subcontractors. Only include the primary contractors' staff here, and do not include subcontractor's staff.
 
Schedule M Part A says corporations need not complete the remainder of the questionnaire but should attach a copy of the certificate of corporate good standing issued by the State of California. Where can I get a certificate?
 
You can print out a certificate for your organization at the California Secretary of State Business Portal at http://kepler.ss.ca.gov/list.html
 
The Schedule N-1 Equal Benefits Ordinance (EBO) declaration form used to have a column in Section C for benefits offered to employees only. What happened to this column?
 

Correct! This is an error. An accurate copy of the Schedule N-1 form is now available.

 
My organization employs youth through a training program. Do we have to pay them the Living Wage as detailed in Schedule N?
 
There are exemptions for youth under 21 who are receiving training and working in temporary positions. Please read the Living Wage Ordinance and call the Contract Compliance office at 510-238-6261 if you have further questions.
 
Do I need to send in supporting documents for the Schedule N-1 Equal Benefits Ordinance (EBO) with my application?
 
No, the signed schedule is sufficient. You will be required to submit the supporting documents if you are awarded a grant.
 
Do I need to send 5 copies of all the City Schedules?
 
Yes please.
 
My organization had a grant from the City of Oakland last year. Can I send copies of all of our schedules and attachments from last year?
 

If the schedules and attachments from last year's grant are still current, you can send us copies. You must review the documents for your organization and determine whether any updates are required.

These documents do not expire unless your agency's compliance or signatory changes:

  • Schedule C1 (ADA Compliance Declaration)
  • Schedule M, Part A (Independent Contractor Questionnaire)
  • Schedule N-1 (EBO Certification)
  • Schedule O (Campaign Contribution Form)
  • Schedule P (Nuclear Free Zone Disclosure)
  • Schedule Z (Certificate of Debarment and Suspension)

The following documents should be reviewed, and probably need an update:

  • IRS Letter of Non-profit Status-- must be within 2 years
  • Schedule D Professional Services Questionnaire-- provide current staffing info
  • Schedule N Living Wage Ordinance Compliance Declaration--please update
