The program ensures maximum independence and autonomy for our
clients. Our care managers are committed to mobilizing any and all
available community supports needed to maintain independent living
and avoid hospitalization and nursing home placement.
Eligibility
Oakland
residents
Age 18 to 100 plus
Frail elderly or adults with disabilities
Need assistance to remain living in the community
Have unmet needs (that do not exceed program’s
capacity)
Be willing and able to participate in the care
management process
