Multipurpose Senior Services Program (MSSP)

MSSP is a comprehensive care management program designed to enable frail seniors to remain safe and independent in their own homes. MSSP is funded by California Department of Aging. You may be eligible if you:

Are 65 or older
Live in Oakland/Emeryville/Piedmont or Berkeley zip codes 94704, 94705
Receive monthly Medi-Cal benefits
Are at risk of nursing home placement because of frailty or inability to manage without help
Are willing and able to accept care management

What to Expect as a Client

A visit by a social worker and a nurse to evaluate you in your home.
A care plan developed with you, your family and other caregivers to address and identify your physical, emotional, social and environmental needs.
Referral to and purchase of services and advocacy, if needed, to help you stay safe and independent in your home.
